For all commercial film production activities occurring on City of Flagstaff owned property, the Film and Media Permit applicant must provide a Certificate of Insurance.

Film and Media Insurance Requirements

  1. Certificate of Insurance for commercial general liability with an endorsement, auto liability (if applicable), and liquor liability (if applicable) naming the City of Flagstaff as additional insured.
  2. Insurance coverage must be maintained for the duration of the filming including all set up and tear down times on city property.
  3. The certificate must indicate the dates, times, and location of the filming. The person/organization listed on the certificate must be the Applicant.
  4. Have insurance providers address the certificate to the attention of the City of Flagstaff Film Office and submit it after the Film Permit Application is approved.
  5. Minimum limits are as follows:
    • $ 1,000,000 liquor liability insurance (if applicable)
    • $ 1,000,000 automobile liability (or non-owned automobile liability) (if applicable)
    • $ 1,000,000 aggregate
    • $ 1,000,000 per occurrence
  6. Workers’ Compensation statutory coverage with basic employers’ liability limits (if applicable)
    • $100,000 per occurrence for bodily injury
    • $100,000 per employee for bodily injury by disease
  7. $500,000 aggregate for bodily injury by disease.
  8. Additional limits may be required after review.
